The effects of L-carnitine on the metabolic alterations induced by rehabilitative respiratory physiokinesiotherapy were evaluated versus placebo in two randomized groups of patients with chronic respiratory insufficiency (pink puffer type). The trend emerging from the preliminary data shows that a 7-day course of treatment with L- carnitine (2 g TID) is capable of preventing the metabolic acidosis and hyperventilation systematically encountered in basal conditions in response to physiotherapeutic treatment. The metabolic phenomena observed are interpreted on the basis of the biochemical properties of the drug, which is capable of stimulating the beta- oxidation of fatty acids and of accelerating the extrahepatic turnover of ketone bodies, thereby sparing the anaerobic glycolytic pathway and leading to a reduced release of free acid radicals.
