OBJECTIVE: To analyze literatures in recent years about rehabilitation training after anterior cruciate ligament reconstruction with autograft, and understand the new developing tendency in this field. DATA SOURCES: Articles between January 1990 and March 2005 were searched in Medline with the key words of 'anterior cruciate ligament, reconstruction, rehabilitation' and the language was limited to English. Meanwhile, Chinese articles between April 2000 and April 2005 were searched in Chinese journal full text database with the key words 'anterior cruciate ligament, rehabilitation, reconstruction'. STUDY SELECTION: Articles related to rehabilitation training after anterior cruciate ligament reconstruction with autograft were collected. Inclusive criteria: (1) randomized or self-control clinical studies; (2) definite standpoints; (3) reconstruction with autograft. Exclusive criteria: (1) reviews; (2) repetitive studies; (3) reconstruction with allograft or synthetic stuff. DATA EXTRACTION: Totally 49 articles about rehabilitation training after anterior cruciate ligament reconstruction with autograft were retrieved, of which 25 in accordance with the inclusive criteria were selected to review. DATA SYNTHESIS: Effects of conservative and jacobinical rehabilitation training on postoperative functional recovery of knee joint were compared and observed. Generally speaking, rehabilitation training of anterior cruciate ligament reconstruction with bone-patellar tendon-bone should be earlier than that with hamstring tendon and training of quadriceps femoris was very important to knee postoperative functional recovery. Closed chain training was suitable for rehabilitation of early stage, whereas open chain training was good to rehabilitation of terminal stage. There was proprioceptor in anterior cruciate ligament, while there was no proprioceptor in stuff used for reconstruction. So, it was essential of proprioceptive enhancing training for functional recovery of knee joint. CONCLUSION: rehabilitation training after anterior cruciate ligament reconstruction is more and more jacobinical. Plan of rehabilitation training should be established according to the stuff of reconstruction and individual differences.
