Despite downward trends in the use of hospital-based services under prospective reimbursement, the hospital remains the major site of health care provision for older people. An evaluation of the impact of discharge planning on patient disposition and rehospitalization by examining modalities of hospital care delivery that emphasize a multidisciplinary team approach is given.
