AIM: To make a systematic review of randomized controlled trails of the physiotherapeutic treatment of whiplash injuries. METHODS: The literature (Index Medicus, Exerpta Medica, Physiotherapy Index) from 1985 to 1993 was searched, using the search words whiplash, cohort study, experiments, physiotherapy. Five randomized controlled trials were found. These articles were blinded by their author and their methodological quality was assessed by two independent researchers, who used a methodological criteria scoring list developed in collaboration with the Department of Epidemiology, University of Limburg. RESULTS: Large differences between the five studies were found with regard to the study description and methodological aspects. Four of the studies scored less than half of the maximum score. A poor description in the publication of what was really measured was an important reason why the studies had a low score on the criteria list. Three studies were able to answer the research questions posed because the study design was inappropriate. CONCLUSION: The quality of future randomized controlled trials on this subject can be improved on the basis of the shortcomings of existing studies. Suggestions for improvement are made with regard to the research population and choice of scoring instruments and effects to be measured, the choice and description of the intervention, and the blinding of assessors.
