A new form of self administered chest physiotherapy consisting of positive end-expiratory pressure employing a face mask with a one-way valve and expiratory resistance of 5 to 20cm with attached tracheal tube connection is introduced (Figure !). Fifteen patients with cystic fibrosis participated in a randomised, controlled clinical trial with PEEP therapy for 15 minutes thrice daily for one month and conventional chest physiotherapy for one month. One patient dropped out of the trial. Lung Function values remained unchanged and did not reveal any significant differences between the two forms of treatment. The quantity of sputum was considerably greater during PEEP therapy. X-Ray of the thorax did not reveal any difference between the two forms for treatment. The advantages of self-administered PEEP are mentioned. The patient is independent of a therapist and treatment is easy and cheap.
