INTRODUCTION: The positive end-expiratory pressure is used in physical therapy. OBJECTIVE: The objective of this study is to search how the EPAP has been used therapeutically, as well as its physiologic effects in several clinics situations. METHODS: This search was performed assessing Medline, LILACS and SciELO databases, in Portuguese and English, with combination of key words related to the subject. Was excluded article about the use of PEEP during mechanic ventilation, non invasive ventilation and respiratory positive pressure intermittent (RPPI). RESULT: Two hundred fifty-seven different studies were found and eighteen were selected for the analysis. The majority of studies were randomized, it used flow resistors and PEEP above 10cmH2O. The EPAP demonstrated to be effective on pulmonary function, gas exchange and desobstruction pulmonary. CONCLUSION: The literature supports good results about the use of EPAP, however, there is not an agreement about the best application form to EPAP. Therefore it is essential to search the standardize of the use this device.
