OBJECTIVE: The pectoralis major muscle promotes anterior chest closure by its active expiratory role in quadriplegia. The purpose of this study was to evaluate the effect of stretching the pectoralis major on the vital capacity of patients with low tetraplegia. METHOD: In the treatment group, passive stretching was performed for 3x30 s for 6 weeks; the control group, the arm was simply mobilized in abduction. The main endpoint was vital capacity, and the secondary criteria were chest expansion and FEV1. RESULT: The treatment group achieved a significant gain in vital capacity p < 0.001). Axillary and xiphoid chest expansion were also significantly enhanced (p < 0.05 and p < 0.01, respectively) after treatment. CONCLUSION: Analytical pectoralis major stretching can improve respiratory function in patients with low tetraplegia. LEVEL OF EVIDENCE: III (low-power randomized controlled trial without inter-group evaluation).
